From Chaos to Clarity: How to Simplify the COC Form Process

What is a Chain of Custody Form (COC) and why does accurately filling out the form matter?

Have you ever watched a forensic TV show where a perfectly prepared case falls apart due to a flawed Chain of Custody (COC) documentation, resulting in the culprit walking free? A COC is a record that documents the collection, transfer, receipt, analysis, storage, and disposal of a sample. At every stage, someone is accountable for the sample as they possess it.

Maybe you have received an incorrect order at a restaurant. It’s usually due to an error in the order. An incorrect COC form can result in vague sample information, and our analysts may not know what needs testing. That’s why it is important to fill out a COC in its entirety.

  1. Client and Project Information
  • The client and project information section are in the upper right-hand corner of the COC. 
  • Franchise Number: Only fill it out if this applies to you or your business.
  • Fill out the Client information (Company, Name, Phone, Billing Address, Email, and Project number)
  • Sample Address: This is where the sample(s) are taken from. 
  • Sample Information

The sample information section is in the middle of the COC, you can’t miss it! 

  • Lab Number and results are for our lab analysts to fill out. 
  • Sample #: Use this to label each sample taken and label bags accordingly. 
  • Room Location: List the location in which the sample was taken.
  • Material Description: List a description of the material you have taken as a sample. 
  • Sampled by: Who took the sample.
  • Relinquished by: Who dropped off the sample or who mailed the sample to Techtron.
